Preventive care is covered at 100% by a network provider.
Immunizations

Talk to your child’s doctor or nurse about the vaccines recommended for their age. COVID-19 vaccination is recommended for some adolescents.
Routine physical examination
One per calendar year for members age 22 and older, limited to:
- Patient history and risk assessment
- Basic metabolic panel
- General health panel
- Well care such as annual counseling for sexually transmitted infections, contraceptive methods, and screenings for interpersonal and domestic violence.

If you’re gender non-conforming, non-binary or on the transgender continuum, your needs may be different. Be sure to talk with your provider about which screenings are appropriate based on your personal situation, including screenings not listed.
